The paper describes the structure of the Health Information Systems course aimed at stu-dents in the 1st year of the Master of Science in Biomedical Engineering at the University of Naples Federico II. Through the extensive use of simulation tools, an attempt was made to implement a project-based learning model for the acquisition of a set of "key" skills forthe training of biomedical engineers that are particularly valuable in the relevant professional area. In the case of digital simulation "didactics," the relationship between realized experience and conceptual structuring becomes central to the learning processes activated. In this sense, the mediation between the simulation process with artifacts-didactic mediation-cognitive growth of the subject achieves a long series of transformations both in the learner and in the tech-nological objects and software used, which are in turn "adjusted" based on the specific needs of those subjects and that context. The same is true for docimological choices adopted such as the use of remote -computer-mediated -examination modes chosen during the violent irruption of the pandemic during A.A. 2020/2021
