Although liver biopsy represents the gold standard to ascertain the ethiopathology of liver disease, it is associated with a significant rate of complications and mortality. This is heightened, in the context of CVID, by the additional infectious risk due to the underlying immune defect.3 Indeed, we suggest that the assessment of liver stiffness by TE should be used alongside clinical, laboratory, and imaging tools to both improve the accuracy of detection of liver disease and select subjects eligible for liver biopsy. Further studies are needed to establish the optimal timing of re-screening, the actual power of this technique to diagnose liver involvement in a preclinical phase, and the degree of concordance between TE results and liver histopathologic findings.
