BACKGROUND: Clinical evidence indicates the involvement of altered insulin activity in acne pathogenesis, both in female than in male. Insulin-sensitizing agents have been used as adjuvants in acne treatment, such as metformin. Clinical and experimental studies highlight important effects of inositol in insulin pathway. As a matter of fact, it is used in the treatment of cutaneous disorders in young women with polycystic ovary syndrome. The aim of our study is to evaluate the efficacy of a supplement of myo-inositol (2000 mg), liposomal magnesium (56.25 mg) and folic acid in male patients affected by acne resistant to common treatment and metabolic profile alteration. METHODS: 30 young male patients with acne resistant to common standard therapies and an altered metabolic profile were selected. The patients were divided into 2 groups:15 subjects who were administered myo-inositol 2000 mg, liposomal magnesium (56.25 mg) and folic acid associated to topical antibiotics and normocaloric diet (group 1) and 15 subjects who were administered the only topical antibiotic and normocaloric diet (group 2). RESULTS: In all patients were performed: GAGS, photographic assessment, anthropometric evaluation, total and HDL cholesterol, glycemia and insulin, OGTT; HOMA-IR was calculated. In group 1 GAGS was reduced statistically significant. Moreover subjects in group 1 at T1 had improved metabolic profile (insulinemia and HOMA-IR decreased statistically significant). CONCLUSIONS: Our study emphasizes the importance of metabolic profile evaluation in acne patients resistant to common therapies. It shows that patients with an altered metabolic profile and insulin resistance can obtain a significant improvement of acne severity with a supplementation of myo-inositol, folic acid and liposomal magnesium.
