A case of 54-years-old woman with a vescico-bullous eruption appeared only on photo-exposed skin areas, after an intense sun exposure, is reported. At same time, she suffered from erosions of the mounth and lips; also, she denied systemic or topical treatments. The results of routine laboratoty findings excluded the possibility of phototoxic and photoallergic reactions. Histopathologic examination, direct and indirect immunofluorescence studies were compatible with the diagnosis of bullous pemphigoid. Provocation testing with UVA (30J/cm2 applied three times at intervals of 24h) led to eruption of blisters in the clinically unaffected area. In contrast, photoprovocation with UVB radiation caused no pathologic reaction. Direct immunofluorescence on UVA-irradiated skin demonstrated linear deposits of IgG at the dermal-epidermal junction. Definitive diagnosis was made of bullous pemphigoid UVA induced.
