On the basis of experience concerning 49 cases of acute pancreatitis, the occurrence and the peculiarities of some signs and symptoms is pointed out: pain, jaundice, circulatory changes, electrolyte disorders, fever, problems related to previous vascular insufficiency of the celiac-mesentery axis, disorders of the gastroenteric, renal and respiratory systems. To evaluate the patients, the role of a correct history and of a careful physical examination is important with a few specific laboratory tests (blood and urine amylasis, amylasis clearance as compared to urea or creatinine clearance, circulating blood levels of lipase, trypsine, elastase and methaemalbumine) which by themselves lead to a proper diagnosis. Many investigations, such as selective arteriography, scanning, upper GI series, cholangiography and Wirsung catheterization are of little help and are sometimes even dangerous. The most common complications of acute pancreatitis are depicted. The more and more actual surgical therapy of acute pancreatitis resistant to medical treatment is stressed.

Abstract
