On the basis of personal experience, a different treatment of ovarian cysts is proposed. In fact, the clinical observation is limited cysts with a maximum diameter of < 8 cms, while the treatment is surgical if the cysts exceed this size. To this purpose, the case of a patient in advanced age (> 75 years) recently observed that also required a surgical approach is described.
