The risk of nonindicated bariatric surgery in patients with undiagnosed Cushing syndrome reported in this paper underline the opportunity to screen for Cushing syndrome in selected obese patients if strong clinical suspicions are present, especially if simple screening tests are used. Moreover, the possibility of avoiding inappropriate and potentially lethal bariatric surgery procedures in 1 of the few reversible cause of secondary obesity and the consequences of the delay in the correct diagnosis and treatment of Cushing syndrome should be considered as adjunctive advantages of Cushing syndrome screening before bariatric surgery.
