Coronary artery fistula (CAF) is a congenital condition characterized by a pathological communication between coronary artery, a systemic vein, or one of the cardiac chambers. Iatrogenic CAF were reported secondary to rupture of coronary aneurysm. Acquired CAF draining into cardiac chambers have been described after AMI. Angiography is the gold standard to discover CAF. We describe the case of a patient who developed a fistula draining into the middle cardiac vein in the posterior interventricular sulcus, after AMI, revealed by CT scan.
