Acne scars represent a real challenge to the dermatologist: several histological and macroscopical distinctive features require different therapeutic approaches. Among the several available classification, Goodman et al. (Goodman GJ, Baron JA - The management of postacne scarring - Dermatol Surg. 2007 Oct;33(10):1175-88 ) differentiate acne scars in: discromic scars, superficial and deep atrophic scars, rolling acne scars, remarkably deep scars with loss of tissue (“deep divots”), small and deep scars (“ice-pick”) and hypertrophic scars, that can evolve in keloids. The medical management of acne scars is based on topical products with salicilyc acid, allantoin and retinyl palmitate. Chemical peelings, based on tricloracetic acid, salicilyc acid and glycolic acid, in various concentration, product a skin chemical exfoliation. For the treatment of hypertrophic scars, it’s been proposed the use of liquid silicone gel, recently approved from FDA. Among new approaches in acne scars handling, needling (skin collagen induction, based in on a mechanical collagen induction) represent an effective and safe technique in a particular way for the treatment of rolling scars in acne.
