Oxygen consumption at peak exercise(peak VO2)is a strong independent predictor of the outcome in congestive heart failure(CHF). RAS inhibition with either ACE or ARB is effective on peak VO2. We evaluated whether mechanisms are similar for the 2 categories of drugs and whether their combination is able to produce a synergistic effect.in the multivariate Cox regression analysis, N, peak VO2 and exercise performance during bicycling and walking tests had prognostic significance in elderly hypertensives with CHF treated with ARB in addition to ACE. For pts unable to tolerate an ACE inhibitor the use of an ARB may confer same advantage

Abstract
